What is the Differential Diagnosis of Failure to Thrive?

Failure to Thrive (FTT) is defined as persistent weight below the 3rd percentile for age in infants and young children. The associated terms “Failure to gain weight” (FGW) or “Lack of Normal Physiologic Growth and Development” are more precise, especially for children who are not below the 3rd percentile but are losing weight over time and/or crossing percentile lines on their growth curve. About 70-80% of FTT cases have psychosocial problems that are associated with the FTT or the primary cause of the FTT. We present a case in which the typical imaging appearances of coalescent peripheral hepatic masses with capsular retraction
contributed to the diagnosis. A positron emission tomography-CT (PET-CT) procedure was performed in staging the disease with
a strong suspicion of coeliac nodal involvement confirmed after laparotomy and histological analysis. Our case is unique because
of the rarity of the disease, the young age of the child, and proven nodal metastases at initial diagnosis. The use of PET-CT
allows better staging at initial diagnosis and thus better management with improved follow-up in these patients.

Celiac disease and autoimmune thyroid disease
What is the long term clinical significance of anti-thyroid antibodies detected during surveillance of patients with celiac disease? In this issue of The Journal, Cassio et al have shown that despite a high prevalence of positive screening tests for autoimmune thyroiditis, the rate of development of clinical hypothyroidism is low. The development of autoimmune markers did not correlate with gluten ingested. In fact, many of the subjects developed thyroid antibodies while adhering to a gluten-free diet. Prevalence and Relative Risk of Other Autoimmune Diseases in Subjects with Autoimmune Thyroid Disease
Conclusion: This is one of the largest studies to date to quantify the risk of diagnosis of coexisting autoimmune diseases in more than 3000 index cases with well-characterized Graves' disease or Hashimoto's thyroiditis. HLA-DQ2 (DQA1*0501/DQB1*0201) is associated with several immune disorders including celiac disease which is caused by an inappropriate T-cell response to gluten. Interference with peptide presentation by HLA-DQ2, for example by the use of peptide blockers, is a possible treatment strategy for such HLA associated disorders. A successful implementation of this strategy will depend on the identification of ligands that bind much better to HLA-DQ2 than the disease related epitopes. Collagenous sprue has traditionally been defined as a small intestinal mucosal disorder characterized by persistent diarrhea, severe malabsorption with multiple nutrient deficiencies and progressive weight loss. Pathologically, a severe to variably severe "flattened" mucosal biopsy lesion with distinctive sub-epithelial deposits in the lamina propria region is detected. Histochemical stains and ultrastructural studies have confirmed that these deposits contain collagens. Often, an initial diagnosis of celiac disease is considered but no continued response to treatment with a gluten-free diet occurs.
